29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Sewer Solutions, LLC located at 1374 Yates Street, Denver, CO 80204: On and Preceding September 20th, 2018, the employer did not ensure that each employee in an excavation was protected from cave-ins by an adequate protective system. Workers were digging and working in a trench which was up to approximately 12 feet deep to tie in to a main sewer line for a new construction multi-family dwelling. The following condition exposed employees to struck-by and caught-in/between hazards. Sewer Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1014978, citation number 1, Item 1, issued February 20, 2015 and became a final order on March 17, 2015, with respect to a workplace located at 1470 Lehigh Street, Boulder, CO 80305.
