29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a) Jobsite: Excavation - The employer did not install a protective system to protect an employee from cave-in hazards while installing a water main line inside the excavation measuring approximately 21' in length, 4' 8" in width, and 8 ft. in depth. Observed on or about 9/21/18.
