29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): (a) Worksite: On or about 9/14/2018, employees were not protected while working in close proximity to the energized electrical service line running into the structure, where the power line was not de-energized or guarded. Richard Mayo was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.416(a)(1), which was contained in OSHA inspection number 930344, citation number 001, item number 01 and was affirmed as a final order on 10/1/2013, with respect to a workplace located at 59 Pierce Street, Westerly RI, 02891.
