29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: On or about September 5 2018, at the workplace, the employer failed to report a work-related fatality to OSHA within eight (8) hours. The employee was struck and crushed by a 2018 Case IH Magnum 280 farm tractor.
