29 CFR 1926.501(b)(1): Employees on walking/working surfaces with an unprotected side and/or edge which is 6 feet or more above a lower level were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: Instance a. 280 Pleasant St. Eastside of building - On or about July 28, 2018, an employee suffered severe injuries as a result of falling from the roof, where fall protection was not provided or enforced by the employer. Instance b. 280 Pleasant St. Southside of building - On or about August 28, 2018, an employee suffered fatal injuries as a result of falling from the roof, where fall protection was not provided or enforced by the employer.
