29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) At 404 Old Post Road, Poughkeepsie NY, on or about August 15, 2018, an employee received a broken wrist, broken ribs, and a punched lung after falling 40' from the top of a scaffolding. The employer did not report the hospitalization to OSHA within the 24 hour reporting period.
