29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about August 23, 2018 and at times prior to, employees perform maintenance and servicing on various equipment, including but not limited to Tenter 1. Employees did not lock out the pneumatic energy of the Tenter #1 while removing jams from the chain and sprocket, cutting contaminated cotton from the roller and deep cleaning or performing servicing on the equipment and were exposed to struck by, caught in hazards.
