29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. The employer has failed to report an employee in-patient hospitalization to OSHA within 24 hours. This most recent occurrence of this took place at the facility located at 72227 578 Ave, Plymouth, NE 68424. On August 13, 2018 the employer has employees installing grain handling equipment on a new set of bins when a piece of horizontal tubing that was being held up in the air by a crane so I could be secured to the existing components fell and struck one of the employees resulting in a hospitalization.
