29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: 241 B Bliss St, West Springfield, MA On August 15, 2018 and for months prior, some powered industrial truck operator employees were overdue their triennial performance evaluations. New Penn Motor Express LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.178(l)(4)(iii) which was contained in OSHA inspection number 962707, citation number 1, item number 3b and was affirmed as a final order on June 25, 2014, with respect to a workplace located at 28 Sterling Rd, North Billerica, MA.
