29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. (a) Facility - On or about July 31, 2018 employer did not report two separate employee amputations within 24 hours. - Separation Room. Caught-in screw conveyor - Separation Room. Caught-in pulley and belt The Cottonseed Co-Op Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number 1260542, citation number 02, item number 001 and was affirmed as a final order on 12/01/2017, with respect to a workplace located at 100 Mill Street, Jonestown, Mississippi 38639.
