29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about July 24, 2018, the employer failed report to OSHA an accident resulting in a in-patient hospitalization within 24 hours. On July 26, 2018 the employer reported an employee's fall resulting in injuries requiring hospitalization that occurred on July 24, 2018.The employer had the option to report the employee hospitalization on-line at osha.gov or by calling the OSHA 24-hour hotline or the Jacksonville Area Office.
