29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, the employer failed to report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: (a) Native Tree Service, LLC., at 9241 High Street in Thornton, Colorado 80229: On or before July 6, 2018, the employer did not report a work-related death to OSHA within 8 hours. On July 5, 2018, an employee sustained an injury while operating a chainsaw in a tree and died on July 5, 2018.
