Location: 77 Clarkson Avenue, Brooklyn, NY. On or about 6/21/2018, a) The employer failed to report a work-related incident resulting in a work related in-hospitalization to OSHA within 24 hours of occurrence. An employee standing on form work fell approximately 9 feet and was hospitalized.
