29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation. On or about June 20, 2018: The employer did not ensure that the 300 Log for 2018 was filled out in detail as required by the standard. (a) For 5/28/18 injury - lifting boxes hand, wrist and forearm are hurting. No other description of the injury was given. (b) For 6/19/18 injury - slipped on a shipping paper packet from one of the boxes. No description of the injury was given.
