29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about June 7, 2018, the employee failed to report an amputation within the 24 hour time period: a) On or about May 22, 2018, Laborer - An employee operating the Reivicart machine, SN: 1330713, received a right hand finger injury resulting in admittance to the hospital for treatment. The incident was reported to OSHA on June 1, 2018.
