29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: a) Outside Engine Room 1; On or about June 12, 2018, and at times prior, the employer exposed employees to fall hazards, in that the 52" wide stair system having 11 risers leading to Engine Room 1 lacked stair rail protection on each open side.
