29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. (a) Iron Innovation, Inc. - On or about May 26, 2018 the employer did not report a work related fatality to OSHA within 8 hours. The employer reported the death on May 29, 2018.
