29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. On or about May 21, 2018, and times prior thereto: a. In the White Production Area, where employees were exposed to fall hazard when filling a cement hopper from a work platform without a self-closing gate on the opened end. b. In the Grey Production Area, where employees were exposed to fall hazard when filling a cement hopper from a work platform without a self-closing gate on the opened end. c. In the Grey Bagging Area, where employees were exposed to fall hazard when filling a cement hopper from a work platform without a self-closing gate on the opened end. SESCO CEMENT, CORP. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.28(B)(3)(IV), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1203550, CITATION NUMBER 1, ITEM NUMBER 5, AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 7, 2018, WITH A FINAL ABATEMENT DATE OF MARCH 9, 2018, WITH RESPECT TO A WORKPLACE LOCATED AT 8510 E. SAM HOUSTON PKWY N. HOUSTON, TX 77044.
