29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a) On May 16, 2018, employees working in an excavation up to approximately 10 feet in depth were exposed to cave-in hazards. b) On May 16, 2018, employees working in an excavation up to approximately 10 feet in depth were exposed to cave-in hazards where shield systems in use were not adequate. A. Lamp Concrete Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 979580, citation 1, item 1, and was affirmed final order on December 23, 2014, with respect to a workplace located at Golf Rd. & Sayre Ave., Morton Grove, IL 60053. No abatement certification or documentation is required for this item.
