29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about May 9, 2018 at 9:30 a.m., the employer reported a workplace amputation that occurred at 1:45 a.m. on May 8, 2018. The report took place more than 24 hours after the incident. No abatement certification or documentation is required for this item. Aspen Products, Inc. was previously cited for a violation of this occupational safety and health standard CFR 1904.39(a)(2), which was contained in OSHA inspection number 1079499, citation number 2, item number 1 and was affirmed as a final order on 10/20/2015, with respect to a workplace located at 4231 Clary Blvd., Kansas City, MO 64130.
