29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) ROOF OF STRUCTURE, EAST SIDE, 3005 MERIDIAN COMMONS, MECHANICSBURG, PA - The employer used a Pettibone 8044 rough terrain forklift to support a scaffold platform that was not designed by the manufacturer for that purpose, thereby exposing employees to injuries on or about May 9, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
