Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about May 1, 2018, the following employees working in trenches were not provided trench protection while working in three (3) trenches at a depth of 5.5', exposing employees to a cave-in: a) An employee was not utilizing cave-in protection while working in the north trench at a depth of 5.5'. b) An employee was not utilizing cave-in protection while working in the middle trench at a depth of 5.5'. c) An employee was not utilizing cave-in protection while working in the south trench at a depth of 5.5'. The Perma-Seal Basement Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 908957, citation number 2, item number 1 and was affirmed as a final order on July 7, 2013, with respect to a workplace located at 532 Turner Avenue, Glen Ellyn, IL 60137. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
