29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer does not ensure all equipment used at the facility is provided with proper machine guarding, exposing employees to amputation hazards. a) This violation was observed on or about April 17, 2018, where an unknown model/in-house designed hydraulic press was not properly guarded. b) This violation was observed on or about April 17, 2018, where an in-house designed stretch bender machine was not properly guarded. Mar-K Specialized Manufacturing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1064329, citation number 1, item number 2, and was affirmed as a final order on August 17, 2015, with a final abatement date of September 4, 2015 with respect to a workplace hazard located at 6625 West Wilshire Blvd, Oklahoma City, OK 73132.
