29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) On or about 4/19/2018 a program planned inspection revealed that a forklift driver's performance was not being evaluated at least once every three years. The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 920649, citation number 2, item number 1, and was affirmed as a final order on 08/29/2013 with respect to a workplace located at 16273 Lower Harbor Road, Brookings, Oregon.
