29 CFR 1904.39(a)(1): Within eight (8) hours after the death of any employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. On or about March 10, 2018, the employer did not report a work related fatality of an employee pinned underneath a 2003 Toyota Tundra weighing approximately 4000 pounds on the southeast side of the automobile salvage yard.
