29 CFR 1926.1204(c)(3): Develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to, the following: Isolating the permit space and physical hazard(s) within the space; a. On or about April 17, 2018, at the construction site located at 5300 North Meadows Drive, Grove City, OH, the employer did not implement the means necessary for safe entry in to a permit required confined space. An employee working in elevator pit #10 was exposed to crushing hazards while working beneath a hydraulic elevator car which was not secured from falling. b. On or about April 18, 2018, at the construction site located at 5300 North Meadows Drive, Grove City, OH, the employer did not implement the means necessary for safe entry in to a permit required confined space. Employees working in elevator pit #12 were exposed to crushing hazards while working beneath a hydraulic elevator car which was not secured from falling.
