29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Site - On or about 04/10/18 and at times prior thereto employees assigned to the installation of a coolant expansion tank powered portable tools with a make-shift extension cord built with a metallic 110V four-outlet receptacle box on the female connection side. The box was not permanently installed according to the original design and intended use, and was left on the floor connected to a flexible cable. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
