29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) United Insulators, Inc. 1206 Osceola Street Denver, CO 80204: On or before March 31, 2018 the employer did not report to OSHA that their employee had been admitted to the hospital. The employer reported the hospitalization about a week later on April 6, 2018.
