29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. (a) At the jobsite - On or about March 22, 2018 an employee troubleshooting the FCM line 5 to remove material stuck at the equipment did not attach a lock nor tag to the hydraulic pump that controls the stock cutter exposing employee to moving equipment. Axiall Corportation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i) which was contained in OSHA inspection number 1186623, citation number 1, item number 3 and was affirmed as a final order on June 14, 2017, with respect to a workplace located at 20043 Hwy 51, Hazlehurst MS 39083.
