29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: The employer is failing to evaluate employees' exposure to respiratory contaminants in the workplace located at 431 A Street, Lincoln, NE 68501. This was most recently documented to occur on 3/19/2018 and at times prior when employees were required to chip-out concrete in the batch mixer without an evaluation of exposure to respirable dust and crystalline silica to determine if respiratory protection was needed.
