29 CFR 1910.217(c)(1)(i): The employer did not provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. a. Toledo Tool & Die Company, Inc. located in Toledo, Ohio: On or about March 23, 2018, the employer did not ensure employees operating the #90 250 ton Komatsu part revolution mechanical power press, were protected from point of operation hazards in that the light curtains were placed 14 inches back from the machine and allowed employees to place their bodies between the light curtains and the press. Toledo Tool & Die Company, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 1910.217(c)(1)(I), which was contained in OSHA inspection number 1096665, citation number 1, item 2 and was affirmed as a final order on March 23, 2016, with respect to a workplace located at 105 West Alexis Road, Toledo, Ohio.
