29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Located at 133 Main Street Salisbury, MA On or about February 23, 2018, the employer did not record the following workplace injury and illness on the OSHA 300 log for calendar year 2018; a) On or about February 23, 2018, an employee fell off of a loading dock and sustained a broken hip. The employee remains out of work to date. Abatement Certification and Documentation is required
