29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about 2/28/2018 at 150 Dundee Drive, Milledgeville, GA: It was determined that the employer failed to report to OSHA a work-related hospitalization within 24 hours of such knowledge on 2/9/2018, and did not contact OSHA until 2/22/2018, at approximately 3:00 pm.
