29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: The employer is failing to protect employees from struck-by and engulfment hazards associated with trenching and excavations. This was most recently documented on or about January 30, 2018 at a jobsite located at or near 16 Hawk Ridge Circle, Lake Saint Louis, MO 63367. The employer did not provide or require the use of benching, sloping, shoring or other means to ensure that employees working in excavations were protected from engulfment hazards.
