29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent: Studio Theatre - On or about January 12, 2018, an employee received an injury to her ankle resulting in being placed on light duty for two weeks and the employer did not use an equivalent 301 form to record the injury for calendar year 2018. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
