29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) The employer's documented hazardous energy control procedures did not outline the scope and purpose of the procedure(s) and the means to enforce compliance with the procedure(s). On or about September 16, 2017, a maintenance employee locked and tagged out the Little Davis tape machine to reinstall the missing guard over the drive chain and sprockets utilizing the defective procedure. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
