29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: The employer failed to report the hospitalization of an employee within (24) twenty-four hours as required. On or about November 30, 2017, an employee operating on of the company's fork trucks was injured after crashing into a guard rail in the Melt Shop due to the throttle pedal sticking. The hospitalization was not report until December 04, 2017.
