29 CFR 1904.39(a)(2): The in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident was not reported within 24 hours. 54 Eastford Road, Southbridge, MA: On or about November 29, 2017 the employer failed to report an amputation as a result of a work related accident within 24 hours. Accident had occurred at 7:20 AM November 28, 2017 date and time of report was 9:35 AM November 29, 2017.
