29 CFR 1926.501(b)(11): The employer does not protect each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels from falling by guardrail systems, safety net systems, or personal fall arrest systems. This violation was most recently observed on 11/28/17, at a jobsite located at 31 Market Street, Ipswich, MA 01938, where employees working walking on a roof at a height greater than seventeen feet above lower level were not protected from falling. Prior violations referred to include a violation of this Occupational Safety and Health Standard which is contained in: William Trahant, JR. Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1003916, citation number 2, item number 1, and was affirmed as a final order on July 27, 2017, with respect to a workplace located at Congress Street, Salem, MA.
