29 CFR 1904.35(b)(2): The employer did not give their employees, former employees, their personal representatives and their authorized employee representatives the OSHA injury and illness records. a) Mercy Health Systems-St Vincents Medical Center located at 2213 Cherry Street in Toledo, Ohio; On or about February 1, 2018 the employer did not provide copies of the OSHA 300 logs to the requesting authorized employee representative by the end of the next business day. A request was made on Febraury 1, 2018 for copies of the OSHA 300 logs for November 2017 and December 2017, as of April 18, 2018 no OSHA 300 logs were received.
