29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: A. Applied Rubber and Plastics, Inc., 100 Skitchewaug Street, Windsor, Connecticut: On or about November 8, 2017, the employer did not record the following workplace injury on the OSHA Form 300 or equivalent for the calendar year 2017: 1. On or about November 1, 2017, an employee sustained a fingertip amputation injury when caught in an unguarded pinch point on the Atom S 530 CN CNC die-cutting machine. The injury resulted in days away from work as well as restricted duty. No OSHA 300 log was maintained. The injury would have been properly recorded with a check mark in Column H (days away from work), the length of time away in Column K (number of days away), the length of time on restricted duty in Column L (number of days restricted/transferred) and a check mark in Column M1 (injury). Abatement Certification and Documentation is required
