29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): A) Matsu Ohio, Inc., located at 228 East Morrison Street, Edgerton, OH 43517: On or about October 18, 2017, the employer did not ensure that the point of operation of the Minster 800 Ton mechanical power press (Press Type SE4-800, Serial # SE4-800-27011) was adequately guarded. There was an unguarded opening on the East side (parts conveyor side) of the Minster 800 Ton press, allowing employees to reach through into the point of operation. Employees were exposed to amputation hazards. Matsu Alabama, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard (29 CFR 1910.217(c)(3)(iii)(f)), which was contained in OSHA Inspection # 899415, Citation number 1, Item number 3, and was affirmed as a final order on 11/02/2015, with respect to a workplace located at 9650 Kellner Road SW, Huntsville, AL 35824.
