Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) On or about October 6, 2017, employees were exposed to amputation injuries in that the C4A infeed assembly was not locked out prior performing unjamming activities. An employee received a partial finger amputation while attempting to clear candy from a rotating cutting knife. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to maintenance tasks the employer did not: � Physically operate energy isolating devices to control energy sources and affix lockout device(s). � Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. � Verify that isolation and deenergiziation of the machine or equipment had been accomplished. Spangler Candy Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(3) which was contained in OSHA inspection number 1093060, citation number 1, item number 2b and was affirmed as a final order on November 18, 2015 with respect to a workplace located at 400 N. Portland St. Bryan, OH 43506.
