29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Located at The Carlstar Group, LLC, McDonough, Georgia: On or about October 17, 2017, the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for calendar year 2017. (a) On or about September 19, 2017, an employee sustained a back strain due to lifting a heavy tire and was transferred from his job duties for 16 days.
