29 CFR 1926.251(b)(3): Job or shop hooks and links, or makeshift fasteners, formed from bolts, rods, etc. or other such attachments were used: (a) AP Construction jobsite C-1 location at the Camden Town Center, 1 Cooper Street, Camden NJ: An employee was hoisting a steel pile with a shop hook. The steel pile came free of the lifting device and landed on an employee, on or about 9/14/2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
