29 CFR 1910.147(c)(7)(iv): The employer did not certify that employee training had been accomplished and is being kept up to date. The certification did not contain each employee's name and dates of training: a) 1100 Lincoln Road Birdsboro, Pa - The employer did not certify that affected employees working on or near machines including but not limited too: Risco Stuffing Machine were instructed on the purpose and use of the energy control procedures. One employee suffered partial amputation of the middle finger due to the unexpected start-up of the machine on or before September 7th, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
