29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Facility, Machine Shop: The Victor lathe #1640B was not provided with a guard over the chuck to protect the operator from flying fragments and rotating parts, on or about 8/31/2017. b) Facility, Machine Shop: The Victor lathe #S206013 was not provided with a guard over the chuck to protect the operator from flying fragments, on or about 8/31/2017. c) Facility, Machine Shop: Alzmetall Drill press was not provided with a guard to protect the operator from flying fragments and rotating parts, on or about 8/31/2017. d) Facility, Machine Shop: CB Super Service, radial drill press No. 4E1045, was not provided with a guard to protect the operator from flying fragments and rotating parts, on or about 8/31/2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. Armadillo Automation Inc. dba Onyx Valve was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection number 1097518, citation number 1, item number 3a and was affirmed as a final order on 9/15/16 with respect to a workplace located at 835 Industrial Highway Suite 4 Cinnaminson, NJ 08077.
