29 CFR 1904.10(b)(4): Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the retest confirmed the recordable STS, the employer did not record the case on the OSHA 300 Log within 7 calendar days of the retest. a) 2017 OSHA 300 Log: When the employee's audiogram revealed an STS in hearing acuity greater than 25 decibels in both ears at the 2000-4000 hz range the case was not recorded on the OSHA 300 Log. b) 2017 OSHA 300 Log: When the employees audiogram revealed an STS in hearing acuity greater than 25 decibels in both ears the case was not recorded on the OSHA 300 Log.
