29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: (a) On August 15, 2017 at approximately 5:53 p.m. an employee fell approximately 20 feet into an elevator shaft from the main lobby on the first floor to the elevator pit floor below; the hoistway door was opened with a hoistway door key while the elevator car was not at the landing. The employee was confirmed deceased at the scene on this date.
