29 CFR 1904.29(b)(3) Each recordable injury or illness was not entered on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: Throughout facility: On or about 8/11/17 the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2017 within seven calendar days: a) Truck Shop: On 2/9/17 an employee suffered a laceration to the left thumb when a drive shaft fell onto his hand when working on a vehicle. The employee received a splint and was placed on work restriction for one week. b) Heavy Equipment Shop: On 5/11/17 an employee was using a portable grinder and a piece of metal flew under his safety glasses and into his eye. The employee was diagnosed with a corneal abrasion and prescribed antibiotics. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
